I've never encountered this with Minecraft before, but something about this texture pack is doing weird things. I've tried 128x and 256x with similar results. The 64x doesn't appear to have the same issue, but I didn't use it for long.

I'm on Win7 64bit, using 64bit java. My computer is an i5 2500k with 8gb of ram and an nVidia 560Ti with up to date drivers. AA/AF are set to application controlled, though even with them forced off it made no difference. I've also tried forcing Vsync and disabling Threaded Optimization and that didn't help either.

I've used both MCPatcher and Optifine's Magic Launcher to no avail. I had also tried using a batch file to run the settings posted in the FAQ (allocating 4gb of RAM), but that didn't seem to help either.

Anyone have any ideas?

To me that looks like your graphics card is overheating.

Generally get those elongated artifacts when that happens.

You can check temps using something like GPU-Z:

My GPU doesn't break 60°C, and runs at the same temp and load levels as just running vanilla Minecraft.

I failed to mention earlier, but I've fully reinstalled Minecraft and Java as well.

It looks like there is either a fault with or overheating GPU like HanFox said
i have experienced this before but i since replaced the laptop that was the problem it was an old computer and it is likely that your GPU is getting old

Im also known as Duck5200 in some far reaches of the internetamabob

I have a i7 2600k, 8GB Ram & GTX 580 SLI.

I don't run minecraft in SLI, I use one card, once it started doing this to me, I tried my second GTX 580, and it was still doing it. Both of my cards where running at normal temps. So my cards where not overheating, and it was doing this with both cards not just one. So I assume its a bug within the nVidia drivers that have yet to be addressed.

I think I know what causes this. It seems if you switch texture packs very often, then this will happen. I have just stuck with the 128x pack for a month or so, and I have not had this issue since.

This is not very much help, but I want you to know your not alone, and your card is not defective.

After screwing around some more, it seems I've made it work.

I had tried messing with the video settings before, but nothing seemed to work. This time though, I enabled mipmapping, and then turned on smooth grass which caused all the terrain to be redrawn, and that fixed it. Anything drawn while I have mipmapping turned on works perfectly, but with it off, strange things happen.

I'm now using the 128x pack, and it looks fantastic. Thanks guys.
