Pretty sure shaders can cause issues with it.

I don't see this happen when I check.

Make sure your OptiFine is up-to-date.

If it is try posting your latest.log from the logs folder (found alongside the resourcepacks folder). Maybe it'll highlight the issue.

You don't explicitly need a credit list to post a patch. It's just nice to do, even if it's just a Google sheet with all the URLs of patch threads in.

The mobs missing textures are from a mod. You'd need to figure that out by pressing F3 and looking at them. It should give info in the debug screen where they are from.

Otherwise you'd have to check your mods and work it out that way.

Generally Minecraft has FPS issues on new cards because Minecraft doesn't support the latest OpenGL. It supports 3.2, first released 3rd August, 2009 (we're up to 4.6 now). I fixed up your file for you. Removed all the unnecessary files, renamed everything to be lower case, fixed the pack.mcmeta ;)
