How to correct instal this addon and does he works with "OptiFine 1.14.4 HD U F4 pre2". If so, how to enable it if I have a lot of your addons. thanks for your attantion

How you have it enabled there is correct, and yes it should work with that version of OptiFine.

Thanks! And one more question. In 3D Blocks Addon "Hay Bale" and "Observer" blocks are not hove 3D model, but other block is normal

I've fixed the Hay Bale in the download, but I'll have to take a look at the Observer tomorrow.
